The Annual Health Questionnaire (AHQ) is the starting point for all Fit for Work assessments and Health Surveillance programmes. This confidential document gathers baseline and annual updates on an employee’s medical status, helping employers proactively identify health changes that may impact workplace safety.

The Employee Journey

1. Distribution
   Issued electronically, either annually or when starting a new role.
2. Completion
   Employee provides details on medical history, current health, lifestyle factors, and work-related concerns.
3. Clinical Review
   Questionnaire returned directly to our Clinical Team for secure review.
4. Triage & Follow-Up
   If concerns are identified, the employee is contacted by a qualified Occupational Health Technician or Nurse for a follow-up consultation.
